Failed Popup Message and HTA launch but process still running

I have noticed recently that the Popup Message has been failing a-lot and even the HTA launch so the OS Build updates have been running in the background and restarting peoples computers without warning etc.
I have attached the LT logs showing it ... I checked the LT script and those lines are configured to exist on failure but it doesn't appear to be honoring that ?
