I have now created the tables, but I still receive the original error at the top of this thread.
Is there a way for me to completely remove Chocolatey, and start again from scratch?

The Chocolatey plugin is a selection of SQL tables, a Script and a Plugin DLL that is loaded into the LT console. Removing all 3 items removes the plugin.
Now if you ignore error and continue does the plugin open up?
If so can you add or change any of the settings? If Yes, then after making all the standard setting changes, if you close and reopen plugin does error return after data is present?
If error still exists, did data save after reopen or is there any missing data anywhere that you had placed there before?
If so send us screenshots of that location in the plugin so we can better understand where your issue is popping up.

The plugin opens up after I click OK on the error, and I can add packages, but once I have added a package, it does not appear in the package list, and nothing that I change within the plugin saves. The error also pops up regularly after I have made any changes.

Have a peek at this log on your local workstation, Sort by time stamp to get the very latest log.
Post that back here, should have SQL errors in it from plugin.

Re: Error When Opening Chocolatey
OK, let's drop all SQL tables starting with plugin_sw_chocolatey, restart the DBagent on LT host and allow it to recreate the tables.
Relaunch the plugin and see if the errors remain or do we get the issue back that it can't rebuild tables?
